Who signs a consent form
A Letter of Consent in MERIDIQ is signed twice. The client agrees to it first, and a practitioner at the clinic countersigns afterwards. Knowing that explains most of the questions clinics ask about consents.
✅ What you can do
- Understand which signature belongs to the client and which to the practitioner
- Choose whether clients sign by drawing or by ticking a box
- Work out why a consent a client already signed still shows as unsigned
✍️ The two signatures
A consent carries two separate signatures, and the article view shows both:
Signature | Who provides it | Where |
|---|---|---|
Client signature | The client | In the clinic, on the Registration Portal, on the Booking Portal, or through a link sent by SMS or e-mail |
Practitioner signature | A member of clinic staff | In MERIDIQ, using the Sign icon on the consent row |
The client always agrees first. The practitioner countersignature is what marks the consent complete.
🖊️ Letting clients draw a signature
By default a client accepts a consent by ticking a box that reads I, {name} hereby accept this consent. You can ask for a drawn signature instead.
- Go to General Settings → Registration.
- Find Use checkbox when signing letter of consent.
- Switch it off to collect a drawn signature, or leave it on for the tick box.
This applies everywhere a client signs, including the portals.
📱 Consents sent by SMS or e-mail
When you send a consent from a client's page, the client receives a link, opens it on their own phone and signs there.
To finish it, open the client, find the consent and click the Sign icon.
🔒 After a consent is signed
A consent that has been signed cannot be deleted. Attempting it returns Cannot remove client letter of consent after signing it.
To withdraw one, use the Cancel icon on the row and add a note. The consent stays in the record marked Cancelled, with your note attached, which is what a clinical record should do.
