Create custom questionnaires

Build your own questionnaires with custom titles, descriptions and a range of question types. Once created, you can use them across the Registration Portal, Booking Portal or send them directly to clients.

✅ What you can do


  • Create a new custom questionnaire with a title and description
  • Add questions with multiple answer types
  • Edit, copy or delete questionnaire titles
  • Reorder questions by dragging and dropping


➕ Create a custom questionnaire


  1. Click Record SystemCustom Questionnaire.
  2. Click the Add button in the top-right corner.

  1. Enter a Title for the questionnaire (required).
  2. (optional) Add a Description explaining the questionnaire.
  3. Click Submit.

➕ Add questions to the questionnaire


  1. After creating the questionnaire, the question editor opens.
  2. Drag the three dots on any question to reorder it.


Questions List


  1. Click New Question to add a question.
  2. Enter your Question text (required).
  3. Click the Type dropdown to choose the answer format:


Type

What it does

Yes/No

Simple Yes or No — if Yes, shows an optional text box

Paragraph

Open-ended text field for long answers

File Upload

Client uploads a file (all, PDF or JPG)

Dropdown

Client selects from a dropdown list

Choices

Client selects one option from multiple choices

Checkbox

Client selects multiple options

Short Answer

Free-text short input

Linear Scale

Client selects a value from a numeric or labeled range


  1. (optional) Toggle Required to make the question mandatory.
  2. Click Submit to save the question.
