Letter of consent setup

Create and manage your Letter of Consent templates — used when sending consent forms to clients directly from their profile or via the Registration and Booking portals.

✅ What you can do


  • Create new letter of consent templates
  • Edit existing consent form content
  • Activate or inactivate consent templates
  • Search and sort consent templates



  1. Click Record System → Letter of Consent.
  2. Click the +Add button in the top-right corner.
  3. Enter the title and content of the consent form.
  4. Fill in all required information.
  5. Click Submit to save.

From the consent list you can:


  • Search — use the Search field to find a consent by name
  • Sort — sort by Recently Added, Alphabetically or Active/Inactive status
  • Edit — click the Edit icon to update the content of a consent form
  • Activate/Inactivate — click the toggle icon to activate or deactivate a template


Letter of Consent Actions


â„šī¸ Inactive consent forms are hidden from the default list. Use the Active/Inactive filter to view them.


âš ī¸ Once a consent form has been signed by a client it is locked as a legal record. Changes to the template do not affect already signed consents.
