Client registration portal

The Registration Portal lets clients submit their personal details online before their first visit. Once submitted, everything saves automatically to their MERIDIQ client record.

✅ What you can do


  • Set which fields are visible and required in the registration form
  • Enable questionnaires and consent forms on the portal
  • Share the registration portal link with clients
  • Let clients register their own information before their first visit


🧭 How to open the Registration Portal


  1. Click Record System in the left navigation menu.
  2. Click Registration Portal from the submenu.

â„šī¸ You can copy the portal link and share it on your website, social media or include it in booking confirmation emails via Custom Booking Text.


📋 Set up mandatory fields


  1. Click Record System → Registration Portal.
  2. Click Mandatory Fields.
  • A list of fields appears (Name, Email, Date of Birth, etc.).
  1. For each field, toggle:
  • View — makes the field visible in the registration form
  • Required — makes the field mandatory for clients to fill in
  1. Check or uncheck the boxes based on your requirements.

â„šī¸ The Name field cannot be changed or removed. If Required is not checked for a field, clients can skip it during registration.



  1. Click Record System → Registration Portal.
  2. Click Questionnaire.
  • A list of Standard Questionnaires and Custom Questionnaires appears.
  1. Toggle the switch to enable (ON) or disable (OFF) a questionnaire.
  2. Click the Eye icon to preview a questionnaire before enabling it.
  3. (optional) Click the List icon next to the Letter of Consent toggle to select which consent forms appear on the portal.
  • Select or deselect consent forms in the pop-up and click Submit.

â„šī¸ Standard Questionnaires are created by MERIDIQ and cannot be edited. Custom Questionnaires can be created and edited in the Custom Questionnaire section.


🔗 What clients see on the registration portal


When a client clicks the registration portal link:


  1. They choose their preferred language.
  2. They fill in the personal information fields you set as visible and required.
  3. They answer any questionnaires you have enabled.
  4. They give consent to the processing of their personal data.
  5. They click Save and Finish.

âš ī¸ Each email address can only be registered once. If a client tries to register with an already registered email they will see: "Please enter a unique email." You can update a client's email from their profile at any time.
