# Manage questionnaires

In questionnaires, you can add a Health Questionnaire, Aesthetic Interest Questionnaire, COVID-19 questionnaire and Custom Questionnaire for a client.

| 📍 **Where to find this:** Clients → select a client → Questionnaires tab

## ✅ What you can do

* Add a Health, Aesthetic Interest, COVID-19 or Custom questionnaire
* View and update answers to any questionnaire
* View uploaded files within a questionnaire
* Download completed questionnaires

## ➕ Add a questionnaire

1. Click the **Clients** menu.
2. Click on the client from the list.
3. Go to the **Questionnaires** section in the client details page.
4. Click the **+Add** button — opens the Add a Questionnaire pop-up.

### Health Questionnaire

5. Click **Health Questionnaire** — opens the View/Update Health Questionnaire pop-up.
6. Fill out the answers and click **Submit**. Click **Cancel** to discard.

### Aesthetic Interest Questionnaire

5. Click **Aesthetic Interest** — opens the View/Update Aesthetic Interest pop-up.
6. Fill out the answers and click **Submit**. Click **Cancel** to discard.

### COVID-19 Questionnaire

5. Click **Covid-19 Questionnaire** — opens the View/Update Covid-19 Questionnaire pop-up.
6. Fill out the answers and click **Submit**. Click **Cancel** to discard.

### Custom Questionnaire

5. Click **Custom Questionnaire** — opens the Custom Questionnaires list.
6. Click the **+** button to add a questionnaire — opens the Custom Questionnaire pop-up.

7. *(optional)* Click the **Three lines** button to view all submitted questionnaires for this client — opens the Added Questionnaires list.

## 👁️ View & download questionnaires

1. Go to the **Questionnaires** section in the client details page.
2. Click the **Files** link on a questionnaire to view or download uploaded files.
3. Click the **Eye** icon to view questionnaire details.
4. Click the **Download** button to download the questionnaire.
