# Clinical record reports

The Record reports section provides a detailed overview of client activity, treatment procedures and form completion across the system for any selected date range — designed to support performance monitoring and data-driven decision-making.

## ✅ What you can do

* View summary metrics for clients, procedures, prescriptions and SMS
* Track new client registrations over time
* Monitor procedures performed by day
* Identify top questionnaires and most-performed procedures
* Analyse client age distribution
* Track notes and prescriptions signed

## 📊 Opening Record reports

1. Click **Reports** in the left navigation menu.
2. Click **Record** from the submenu.
* The **Reports — Record** page opens with eight data sections.

## 🔽 Filters

| Filter | Options |
| ---- |
| **Team Member** | Select a specific team member or **Everyone** |
| **Period** | Daily, Weekly or Monthly |
| **Start Date / End Date** | Set a custom date range |
| **Export** | Click **Export** (top-right) to download the report |

## 📋 Report sections

### 1. Summary Metrics
* **Total Clients** — all registered clients
* **Total Procedures** — procedures completed
* **Prescriptions Signed** — signed prescription records
* **SMS Sent** — total SMS messages sent

### 2. New Client Registrations
Bar chart showing the number of new client registrations by day within the selected period.

### 3. Procedure Performed
Line graph showing how many procedures were performed each day.

### 4. Top Questionnaires
Displays the most-completed questionnaires — ranked by Questionnaire Name and Completed Count.

### 5. Most Procedures
Displays the most-performed procedures — ranked by Procedure Name and Completed Count.

### 6. Client Age Distribution
Pie chart breaking down clients by age group: 21–25, 26–30, 31–35, 36–40, 41–45 years.

### 7. Most Notes
Displays the most-added notes — ranked by Note Name and Added Count.

### 8. Prescription Signed
Horizontal bar chart showing prescriptions signed via the MERIDIQ system vs. the MAL platform.
