# Add notes & upload files

Create signed notes, attach files and link prescriptions to a client's record. Notes follow the same sign-to-lock workflow as procedures.

| 📍 **Where to find this:** Clients → select a client → Records → Notes & Files tab

## ✅ What you can do

* Create notes with a title and text template
* Upload files and documents to a note
* Attach prescriptions to a note
* Sign, edit, view, download or cancel notes

## ➕ Add a note & file

1. Click the **+Add** button — opens the New Note pop-up.
2. Enter a **Title** (required).
3. *(optional)* Select a **Text Template** from the dropdown.
4. *(optional)* Click **Add Prescription** to attach one or more prescriptions.
5. *(optional)* Click **Choose files** to upload documents or images.
6. *(optional)* Tick **Custom date & time** to set your own date instead of today.
7. Save or finalise:
* **Submit Draft** — saves without signing, stays editable
* **Submit and Sign** — opens the Sign Note pop-up to lock the record

||| **⚠️ Once signed, the note is locked.** It can only be viewed or cancelled — not edited. You can only download after signing.

## ⚙️ Manage notes & files

| Icon | Action | What it does | Available when |
| ---- |
| ✏️ | **Sign** | Finalise & lock the note | Unsigned only |
| 📝 | **Edit** | Modify the note | Unsigned only |
| 👁️ | **View** | Read-only view | Any status |
| ⬇️ | **Download** | Export as PDF | Signed only |
| 🗑️ | **Delete** | Remove permanently | Unsigned only |
| 🚫 | **Cancel** | Cancel & add a reason note | Signed only |
| 🗒️ | **View cancel note** | Read the cancellation reason | Cancelled only |
